2009 Request Form

To receive your free street tree(s), please fill out this short form and check the box below the Street Tree Care Pledge to acknowledge that you agree to and understand your commitment when participating in the street tree program.

Tree planting is on a first-come, first-served basis. Trees are available in limited quantities.

Street Tree Care Pledge:
I will plant my street tree(s) as specified on this pledge, and on the planting guide. I will plant and leave my trees in my front planter strip. I will water my tree(s) during the summer months for at least the next three years. I will carefully watch over my tree(s) and let the City know if I think there is a problem. I will not use "weed and feed" fertilizer around my tree(s). I will keep weeds and grass from growing in the mulch bed around the tree(s). I will not use a "weed whacker" or lawn mower near my tree(s), as they are the number one killer of young trees. I will not plant flowers or plants within the mulch bed. I will not "top" the tree(s).
