Emergency Medical Cards: Help Us, Help You! 

When you have a medical emergency it is important responders know about pre-existing medical conditions and current medications.  Having this information ensures you receive the best care possible, in the shortest amount of time.

Complete an AFD Emergency Medical Card and post it on your refrigerator.  This card provides our responders with critical information such as current medications, physician's name, past surgeries, and emergency contacts.  You may also include the location of advanced directives including Do Not Resuscitate (DNR) orders.

Fire Incident Reports

Request for Copy of a Fire Incident Report (pdf form)

Use the link above to open and submit a request form. When Albany Fire Department staff receives the request, they will determine if a report is available, calculate the cost of providing a copy of the report, and contact you with instructions for submitting payment.

The cost varies depending on the size of the report ($7 - $25 average). The fees are allowed under ORS 192.440(3).

Fire Station Visits

AFD will accommodate visits to fire stations for small groups. No minimum number of visitors is required.
