On July 9, 2021, about 8:51 p.m. PDT, an experimental amateur-built North Wing Mustang 3 weight-shift-control trike was substantially damaged when it was involved in an accident near Millersburg. The noncertificated pilot and passenger were fatally injured. The trike departed from the Albany Municipal Airport as a Title 14 Code of Federal Regulations Part 91 personal flight.

The final report of the National Transportation Safety Board (NTSB) identifies a left banking turn of 87 degrees as the cause of a stall and subsequent ground impact. The manufacturer’s maximum angle of bank is 60 degrees, and operators are warned by the manufacturer against ever exceeding this limit. There was no evidence of engine or airframe malfunction, or any other contributing cause.
