A City of Albany representative will contact you via phone or email to finish setting up your account and confirm the service start date. Services are established Monday - Friday (excluding holidays) with one business day notice required.
An activation fee of $20.00 will appear on your first billing statement. Completing and submitting this form does not guarantee services nor should it be considered confirmation that an account has been established.
